It loosens with time but it never comes to the point of rendering the cuff useless. The bag is a tight fit for the cuff and the little side pocket is nice for having small alcohol pads, band-aids or a pen. Only gripe is the bag can be awkward to carry around on a belt, but still is a good case until one can get a bag meant to be on a person in the field to carry all the goods.
